The TCP have commissioned a hospital passport, along with supporting guidance. We would like to thank the CHCP wellbeing team and patients and carers who have led on this development, alongside other partners.
The documents are available within the Health and Wellbeing section of the website here for anyone to use and share. We have received some great feedback, and the passport is going to be used by a wide range of services, not limited to people with a learning disability or autism.
Please share within your own organisation and any wider networks who may find the passport useful.
