The Humber Transforming Care Partnership is looking to fund various projects around the Humber region.

The Humber Transforming Care Programme aims to improve health and care services, so that more people with a learning disability and/or autistic people can live in the community, with the right support. This means that fewer people will need to go into hospital for their care.
We have worked with a group of people with lived experience of learning disability and/ or autism to develop a shared vision:
“In the Humber we want autistic people and people with a learning disability to lead happy and healthy lives in communities, where skills, strengths and talents are recognised and valued by all”.
The Humber Transforming Care Programme is hosted by East Riding of Yorkshire Health and Care Partnership, which is part of the NHS Humber and North Yorkshire Integrated Care Board. This grant programme runs until 31st March 2024 .
We would like to fund projects for people (of any age) who have a learning disability/ and or autism, or their family/ carers which will provide opportunities for either social inclusion, physical and/ or mental wellbeing.
