The reviews took place in 2021/22 as part of the NHS response to the safeguarding adults review (SAR) concerning the deaths of Joanna, Jon and Ben at Cawston Park. Integrated Care Boards have been taking action since the reviews have taken place.
This report is a thematic analysis which has been used to inform the Dynamic Support Register and Care Education and Treatment Review Policy and Guidance and to inform the work of the Mental Health, Learning Disability and Autism Quality Transformation Programme. Learning from the reviews will continue to be used to shape the work being delivered and any further guidance.
